Substance Use Disorder (Secondary)

A pattern of substance use causing significant impairment. The VA can service-connect substance use disorder only when it is secondary to (caused or aggravated by) a service-connected condition such as PTSD or chronic pain. Rated under the General Rating Formula for Mental Disorders.

Rating Levels

Your condition causes total impairment in your ability to work and function socially. You may be a persistent danger to yourself or others, disoriented, or have severe memory loss.

What the VA looks for

Gross impairment in thought processes or communication
Persistent delusions or hallucinations
Grossly inappropriate behavior
Persistent danger of hurting self or others
Inability to perform activities of daily living
Disorientation to time or place
Memory loss for names of close relatives, own occupation, or own name

Common Pitfalls

⚠Substance use disorder cannot be directly service-connected — it MUST be secondary to another service-connected disability. Get a medical opinion specifically stating the substance use is caused or aggravated by your service-connected condition.
⚠Not documenting that substance use began or worsened after the onset of the primary condition — treatment records showing the temporal relationship between the primary condition and substance use are critical.
⚠Not providing buddy statements from family or friends who can describe how substance use started or escalated after your service-connected condition developed.
⚠Failing to show ongoing treatment for both the primary condition and the substance use disorder — the VA wants to see that you are addressing the underlying cause.
