Hydronephrosis

Swelling of the kidney caused by a backup of urine. This happens when urine cannot drain from the kidney to the bladder because of a blockage. The VA rates hydronephrosis based on how often you have attacks of colic (severe pain), whether infection is present, and whether kidney function is impaired.

Rating Levels

You have frequent attacks of colic with infection (pyonephrosis) and your kidney function is impaired. This is the maximum schedular rating for hydronephrosis.

What the VA looks for

Frequent attacks of colic with infection
Pyonephrosis documented on imaging or culture
Impaired kidney function on lab work (elevated creatinine, decreased eGFR)
Urology treatment records
Possible surgical intervention (stent, nephrostomy)

Common Pitfalls

⚠Not documenting every episode of colic pain, including emergency room visits — the VA rating depends on whether attacks are occasional or frequent.
⚠Not getting urine cultures during episodes to prove infection is present — the 30% rating requires documented infection (pyonephrosis), not just pain.
⚠Not tracking kidney function with regular lab work (creatinine, BUN, eGFR) — declining kidney function supports a higher rating and may qualify for a separate kidney disease rating.
⚠Not filing for the underlying cause of hydronephrosis as a separate condition — kidney stones, tumors, or prostate enlargement causing the blockage may each be separately ratable.
⚠Not requesting imaging studies during symptomatic episodes — CT scans or ultrasounds showing the severity of kidney swelling during an active episode provide stronger evidence than imaging done between episodes.
